Special Needs Assistance Request Form

Retrieval of Books and Journals

Individuals with disabilities that make it difficult to retrieve books and journal articles are encouraged to request assistance by contacting the library a day or two in advance. The preferred method of submitting these requests is to use the online Special Needs Assistance Request Form. Requests are also accepted by faxing in a copy of the completed copy form, or by telephoning the Reference Desk and asking the Desk staff to complete an Assistance Request Form.

Library staff will make a best effort to search and retrieve a limited number of items within a reasonable amount of time, however staffing and activity levels within the library may prevent us from completing requests on a walk-in basis. All requests submitted using the Assistance Request Form should be available for pickup in less than two business days. An email notice will be sent out when your materials are ready for pickup. An experimental program where circulating materials may be shipped directly to your home using UPS is also available. The cost for this service is $ 7.50 per item.
